Dále se objevovaly jednotlivé uniklé junkční a různorodě vázané polymorfní komorové extrasystoly.

With the word "uniklé", I would expect to see the word "stahy", but it isn't there (in other words, "uniklé" seems to refer to "extrasystoly").

Ano, to slovo tam chybí. Když to přepíšete jako vloženou větu, je to hned srozumitelnější, ačkoli pravděpodobně gramaticky pochybné...
Dále se objevovaly jednotlivé uniklé junkční -a různorodě vázané polymorfní komorové- extrasystoly.

My difficulty here is that there seems to be a word missing in Czech. "Uniklé junkční stahy" would make sense.

It is possible to use the term extrasystoles in the above translation instead of systoles, but IMO the term "escape systoles" already expresses the fact that these are not regular systoles.
Example sentence:

atrioventricular (AV) junctional escape beat a depolarization initiated in the atrioventricular junction when one or more impulses from the sinus node are ineffective or nonexistent

In Czech, the verb "uniknout" means "to get loose", "to escape", "to get away". In this case, however, I would use the word "to leak" (it is being used when talking about gases, liquids etc.). And since "uniklé" is an adjective, the correct translation should be "leaked" (at least in my opinion).
