The option "transportation service (for the elderly or disabled)" has long been the term used in translations from Swedish, but I would agree that what it actually is corresponds more with "paratransit".

Yes, SL (Stockholms Länstrafik) seems to use both:

"Brief information in English about Färdtjänsten in Stockholm County, (Paratransit Service ), a service for those who are unable to use normal public transport."

Paratransit

This would be the most elegant term in English. Not sure if you're looking for UK or US usage.

hospital or community transport

in UK this depends on if the transport is paid for by health or community care funding

In the first occurrence. This is more specific and to the point than just "paratransit".

In later occurrences, where the meaning is clear from the context, just "paratransit" will be fine, e.g. in "färdtjänstutredaren" as in your quote above.
