Context:
Paciente acude en ayunas. Refiere dolor abdominal. Niega estar recibiendo otra medicación y continua (or continúo) con IP idem (or iden, not sure what it say...)( It is hard to read at times).

I cannot state a high degree of certainty because of the lack of more context, however, it seems based on the persistent abdominal pain that the IP might refer to peritoneal irritation which is a sign evoked by the examining physician upon palpating a tender abdomen.
