Does anybody know the specific translation of this document? It seems to be along the lines of a Repair Acceptance Slip, which accepts a claim so that an insured vehicle damaged in an accident can be repaired. I wonder if there's a more precise translation though.

"VOLANTE DE ADMISIÓN
VÁLIDO POR 7 DÍAS HÁBILES

Información del Asegurado
Siniestro No.
Folio etc"

It also includes diagrams of the damaged vehicle (before and after the accident).

Repair authorisation form I think you are on the right lines, but perhaps the above term is more appropriate, if the form is intended to be sent to the repair garage that has been chosen to carry out the (pre-agreed) repairs.

Repair authorisation form

If the form is intended to be sent to the garage selected to carry out repairs, this would seem an appropriate term, as mentioned in my discussion post.

Equally, it could well be a form that the insured signs to agree to the extent (and cost) of repairs to be carried out.

The context does not really show the scenario, but either way I think the above term would work.

Car claim notifications form

Really researched this one ;) pls see below thanks, now this can be accepted (and depending on your policy car be repaired, you get a replacement car etc...or no)
