Dear Ms. Tahirkheli:
I hope this letter finds you well. Our office has been trying to reach you, but none of the phone numbers we had for you or your sister Neelo have worked. I have also tried sending a letter to Neelo, but I have not heard from either of you. It is very important that we talk with you! DSS has requested additional documentation for your Medicaid application. They have sent a second notice, which states your deadline to provide these documents is February 10, 2016. If you do not give them these documents, your application for Medicaid will be denied. The documents they are requesting are the following:
1. Your life insurance policies (or the name of the insurance company and policy numbers
2. Your proof of citizenship and identity
3. Your proof of state residence
4. Your signature on the enclosed forms
Please contact us immediately for assistance in providing this information to DSS. You can reach me at (336) 398-1706 or Miriam Heard at (336) 398-1702. Ms. Heard will also be at the Triad Adult and Pediatric Medicine clinic on Monday, February 8th. You could also call your DSS caseworker Shontae Crouch at (336) 641-3110. If we do not hear from you by February 15, 2016, we will have no choice but to close your file with this office.

2. آپ کی شہریت اور شناخت کا ثبوت
