Source text - English Radiation exposure sensitivity
Radiation exposure is a concern in both adults and children. However, children are more sensitive to radiation than adults and have a longer life expectancy. Radiation risk is higher in young patients, as they have more rapidly dividing cells than adults. The younger the patient, the more sensitive they are. Using the same exposure parameters on a child as used on an adult may result in larger doses to the child. There is no need for these larger doses to children, and CT settings can be adjusted to reduce dose significantly while maintaining diagnostic image quality.
The National Cancer Institute and The Society for Pediatric Radiology developed a brochure, Radiation Risks and Pediatric Computed Tomography: A Guide for Health Care Providers, and the FDA issued a Public Health Notification, Reducing Radiation Risk from Computed Tomography for Pediatric and Small Adult Patients dated November 2, 2001, that discuss the value of CT and the importance of minimizing the radiation dose, especially in children.
Dose reporting considerations
It is widely understood and accepted that adult techniques should never be applied to small patients or pediatrics since smaller objects have higher dose at the same technique. The chart below illustrates the sharp increases in relative dose as the part scanned gets smaller in size using the same technique.

Another consideration about dose is since it is not possible to characterize dose given to individual patients, the CT dose indices are provided to help make relative comparisons. These dose index values can be used to compare CT systems and to help select appropriate operating conditions for scanning. However, it is important to recognize that the dose reported by these indices is inversely proportional to phantom size (see the above chart). This means that for the same scan technique, smaller phantoms (patients) will produce a higher absorbed dose than larger phantom (patients). Therefore, it is critical to remember that the body filter uses the 32 cm CTDI phantom and the head filter uses the 16 cm CTDI phantom for dose reporting purposes.
In other words, when looking at the actual absorbed dose to the patient, understand that the dose may be higher than reported if the part scanned is smaller than the phantom tested.

Il National Cancer Institute e la Society for Pediatric Radiology hanno realizzato un opuscolo, “Rischi da radiazioni e tomografia computerizzata in pediatria: una guida per le aziende sanitarie”, e l’FDA ha pubblicato una Notifica per la salute pubblica, “Riduzione del rischio da radiazioni della tomografia computerizzata per pazienti pediatrici e pazienti adulti di corporatura piccola”, datata il 2 novembre, 2001, che parla del valore della tomografia computerizzata e dell’importanza di ridurre al minimo la dose di radiazioni, soprattutto nei bambini.
Considerazioni relative al rapporto sulla dose
È ampiamente compreso e accettato che le tecniche per gli adulti non devono mai essere applicate a pazienti di corporatura piccola o pediatrici dato che oggetti più piccoli presentano una dose più alta con la stessa tecnica. Il grafico sotto illustra gli incrementi netti nella dose corrispondente quando la parte esaminata diventa più piccola, utilizzando la stessa tecnica.

Un’altra considerazione sulla dose è che, non essendo possibile individuare la dose erogata a singoli pazienti, gli indici di dose della tomografia computerizzata forniscono supporto per fare i relativi raffronti. Questi valori dell’indice di dose possono essere usati per confrontare i sistemi di tomografia computerizzata e per aiutare nella scelta delle condizioni adatte per effettuare l’esame. Tuttavia, è importante essere consapevoli che la dose riportata da questi indici è inversamente proporzionale alla taglia del fantoccio (vedere il grafico sopra). Questo significa che a parità di tecnica di esame, fantocci più piccoli (pazienti) produrranno una dose assorbita maggiore rispetto a fantocci più grandi (pazienti). Perciò, è fondamentale ricordare che il filtro corpo utilizza il fantoccio CTDI da 32 cm e il filtro testa usa il fantoccio CTDI da 16 cm per il rapporto relativo alla dose.
In altre parole, quando si osserva l’effettiva dose assorbita dal paziente, occorre tenere presente che la dose può essere più elevata di quanto riportato, se la parte esaminata è più piccola del fantoccio sottoposto a esame.

The counterparties to these agreements consist of a diverse group of financial institutions. The Company continually monitors its positions and the credit ratings of its counterparties and adjusts positions as appropriate. The Company did not have significant exposure to any individual counterparty at December 31, 2007 and has master agreements that contain netting arrangements. Some of these agreements also require each party to post collateral if credit ratings fall below, or aggregate exposures exceed, certain contractual limits. At December 31, 2007 and 2006, the Company was required to post collateral of $54.8 million and $49.3 million, respectively, which was used to reduce the carrying value of the derivatives recorded in other long-term liabilities.
