A retrospective 5-year study
Abstract: Purpose: The aim of this retrospective study was to analyse bone level alterations
over a 5-year period at implants in the maxillary posterior segments in patients with varying
experience of periodontal bone loss in the natural dentition before implant placement.
Material and methods: 97 partially dentate patients with a total of 346 BrånemarkAoral
implants in the maxillary posterior segments were included. By assessing the degree of
radiographic marginal bone loss in the remaining natural dentition at time of the implant
therapy, an age-related bone loss score (ArB-score) was calculated for description of the
patient’s experience of periodontal destruction. The two end quartiles of the distribution of
the subjects with regard to the ArB-score were defined as Non-Perio subjects and Perio
subjects, respectively. The primary outcome variables were implant losses (implant failures)
and radiographic peri-implant bone loss over the 5-year observation period.
Results: A total of 18 implants were lost during the 5 years, resulting in an overall failure rate
of 5.2%. The corresponding failure rate was 3.3% for the Non-Perio and 8.0% for the Perio
patients. The peri-implant bone loss from the time of abutment connection to 5 years
averaged 1.8 mm (SD 0.7). Of the patients, 34% showed a mean bone loss of 2 mm and 39%
of all implants had experienced a bone loss of 2 mm at the 5-year examination. The Non-
Perio and Perio patients showed a mean bone loss of 1.7 mm (0.8) and 2.2 mm (0.8),
respectively. Multiple regression analysis revealed a statistically significant relationship
between the ArB-score and the peri-implant bone level change from abutment connection to
5 years (P 0.05). In all, 64% of Perio patients had a mean peri-implant bone loss of 2 mm
from the time of abutment connection, compared to 24% for the Non-Perio patients (P 0.01).
The percentage of implants showing 2 mm of bone loss between abutment connection and
5 years was 62% and 44% in the Perio and Non-Perio groups, respectively (P Ω 0.055).
Conclusion: The results indicate that longitudinal bone loss around implants is correlated to
previous experience of loss of periodontal bone support and that periodontitis susceptible
subjects may show an increased implant failure rate.
Discussion
The results of the present 5-year retrospective
study demonstrated that
O the amount of longitudinal peri-implant
bone loss was related to pretreatment
experience of loss of periodontal bone
support, and;
O the overall implant failure rate was
higher for ‘periodontitis-susceptible’
subjects (8%) than for ‘nonperiodontal’
subjects (3%).
Due to the retrospective design of the present
study the classification of the patients
with respect to their experience of periodontal
disease could be based only on preoperative
radiographic data describing the
amount of bone support at remaining
teeth, since clinical data regarding the actual
periodontal conditions at time for implant
therapy, or at the 5-year follow-up,
were not retrievable. Using the periodontal
bone level data and the age of the patient,
an ‘age-related bone level score’ (ArB score)
was calculated for description and stratification
of the subjects with regard to degree
of ‘susceptibility for periodontal destruction (disease)’. The feasibility of the calculated
ArB score for characterization of the
subjects was evidenced by marked differences
between the two tail quartiles of the
patient sample in terms of mean periodontal
bone level (92% vs. 63%) and
mean percentage of teeth with a remaining
bone support of less than half the root
length (1% vs. 26%). In an epidemiological
study by Hugoson et al. (1998), in which
subjects were grouped according to experienced
periodontal bone loss, subjects
showing an overall bone loss of more than
1/3 of the root length were characterized as
having ‘severe periodontal disease’. Accordingly,
it seems justified to consider the patients
belonging to the worst quartile (Perio
group), based on the ArB score, as subjects
susceptible to periodontitis. In the
interpretation of the results of the present
study one may also keep in mind that although
the Perio subjects showed a mean
number of remaining teeth that was the
same as for the entire sample (16 teeth), the
more pronounced bone loss in the natural
dentition may have entailed differences in
loading conditions for the implant supported
fixed partial prosthesis.
The overall implant failure rate (implant
losses) for the 5-year period was in the present
patient sample 5.2%, a figure that is
comparable to published treatment outcome
data for partially dentate individuals
with corresponding follow-up periods (Naert
et al. 1992; Jemt& Lekholm 1993; Nevins
& Langer 1993; Lekholm et al. 1994,
1999; Bahat 2000; Behneke et al. 2000).
Lindh et al. (1998) presented a meta-analysis
of 7 studies including a total 923 implants
supporting fixed partial dentures
and reported a 5-year failure rate of 6.4%.
The outcome of implant therapy in periodontally
compromised patients has been
addressed in a few case series (Nevins &
Langer 1995; Ellegaard et al. 1997). Ellegaard
et al. (1997) reported an implant failure
rate of 5% during an average of 3years
in patients who had received single or partial
implant-supported prosthetic reconstructions,
while a study by Nevins &
Langer (1995) showed an overall implant
failure rate of 3% for various types of implant
supported prostheses in a patient material
followed for 1–8years. In our study,
evidence of previous experience of destructive
periodontal disease was not included
as a criterion for patient selection. The target
population was all patients who during

a defined period of time had received treatment
with an implant supported fixed partial
denture in the posterior segments of
the maxilla at the Brånemark Clinic, Göteborg,
and the characterization of the patients
according to degree of periodontal
disease ‘susceptibility’ was performed first
after inclusion. Hence, in contrast to the
studies referred to above, our patient
sample comprised large variability with regard
to previous experience of destructive
periodontal disease. Interestingly, the data
revealed that the quartile of the subjects
who showed the highest ArB score (Perio
group) demonstrated an implant failure
rate of 8%, compared to 4% for the rest of
sample and 3% for the Non-Perio subgroup.
The finding of a higher implant failure
rate in the Perio subgroup corroborates
results recently reported by Brocard et al.
(2000), showing significantly lower success
rate for a subgroup of ‘periodontally maintained
patients’ than for their entire patient
sample. In our study, the difference was
particularly evident for late failures, i.e. implants
lost after the first year of loading.
Out of a total of seven implants lost in the
entire patient sample after the first year of
loading, five implants (four subjects) belonged
to the quartile of the patient sample
that had the greatest experience of loss of
periodontal bone support. Although one
should be cautious in the interpretation of
these findings due to the low overall frequency
of adverse events, the fact that all
but one of the lost implants had a length
of 13–15mm does not speak in favour of
limited bone quantity as a reason for the
higher implant failure rate observed in
‘periodontitis-susceptible’ subjects.
The peri-implant bone loss from time of
abutment connection to the 5-year followup
was in the present patient material on
the average 1.8mm, out of which 0.7mm
occurred between abutment connections
and loading (bridge insertion). Hence, the
bone loss during the 5-year of loading averaged
1.0mm, a figure that is in accordance
with findings reported in other studies on
free-standing implant supported prostheses
in the maxilla (0.8mm; Jemt & Lekholm
1993; Lekholm et al. 1994, 1999). Although
the overall mean peri-implant bone loss in
the present patient sample may be considered
to be of a limited magnitude, our
data disclosed a statistically significant correlation
between the degree of peri-implant
bone loss and experienced periodontal bone
loss in the natural dentition before the implant
therapy. In fact, the individuals who
belonged to the quartile with the most severe
ArB score showed about 30% greater
mean loss of peri-implant bone support
during the 5years of loading than the rest
of the patient sample. Whether the rate of
deterioration had been greater also in their
remaining natural dentition could not be
evaluated since no follow-up radiographs of
the teeth were available for the present
analysis. Furthermore, due to lack of retrievable
pertinent clinical data, the question
whether adequate periodontal/peri-implant
supportive therapy for the control of
plaque-induced lesions had been provided
could not be determined. The influence of
smoking on the rate of peri-implant bone
loss, as demonstrated in studies by,
e.g.Weyant & Bert (1993) and Lindquist et
al. (1996), could neither be evaluated since
information on smoking habits had not
been routinely recorded. Hence, the observed
relationship between previous experience
of periodontal tissue breakdown
and peri-implant bone loss warrants
further evaluation in prospectively designed
longitudinal studies.

5 yıllık bir geriye dönük çalışma*
Özet: amaç: Bu geriye dönük çalışmanın amacı, değişik düzeylerde periodontal kemik kaybına sahip hastaların maxillar posterior segmentlerindeki implantlarinin kemik düzeyi değişikliklerini implantlar yerleştirilmeden önceki 5 yıllık bir süreçte analiz etmektir.
Materyal ve metotlar: Çalışmaya 97 kısmi dişli hasta, toplamda ise 346 branemark maxillar posterior segmentlerinden implantli hasta dahil edilmiştir. Tedavi sürecinde, doğal diş yapısındaki marjinal diş kaybı radyografik olarak derecelendirilerek yaşa dayalı bir kemik kaybı skoru (ArB score), hastanin geçmiş periodontal zararının tanımlanması için hesaplanmıştır. ArB skoru baz alınarak örneklerin son iki çeyreği Non perio ve perio grupları olarak adlandırılmışlardır. 5 yıllık bir izleme periyodunda edinilen birincil sonuçlar implant kayıpları ve radyografik peri-implant kemik kayıplarıdır.
Sonuçlar: 5 yıllık periyotta %5,2 lik bir kayba eşit toplam 18 implant kaybedilmiştir. Bu sonuca tekabül eden Non perio ve perio kayıp oranı sırasıyla %3,3 ve %8,0 olmaktadır. Bağlantının yapıldığı 5 yıllık süre içinde peri-implanttaki kemik kaybı 1,8 mm olmuştur. Hastalardan %34 u ortalama 2mm den büyük kemik kaybı gösterirken tüm implantlarda 5 yıllık sürede 2mm lik kemik kaybı gözlemlenmiştir. Non perio ve perio grupları ortalama olarak sırasıyla 1,7 mm ve 2,2 mm lik kemik kaybı göstermiştir. Çoklu regresyon analizi 5 yıllık süreçte Arb skoru ve peri-implant kemik düzeyi değişikliklerinde istatistiksel olarak önemli bir bağ açığa çıkarmıştır. Toplamda ise perio hastalarının %64 u 2mm den büyük bir ortalama peri-implant kemik kaybına sahipken Non perio hastalar bu kaybı %24 lük bir oranda yaşamıştır. Perio ve Non perio gruplarındaki bağlantı ve 5 yıllık periyot arasında geçen sürede 2 mm lik kaybın yaşanma sikliği sırasıyla %62 ve %44 tur.
Sonuç: Sonuçlar göstermektedir ki implantlarin çevresindeki dikey kemik kaybı daha önceden yaşanılmış periodontal sorunlu ve periodontite hassas hastalarda fazla olmakla beraber bu hastalarda daha yüksek oranda implant kayıpları yaşanmaktadır.
Tartışma: Bu 5 yıllık geri çalışmanın sonuçları göstermiştir ki:
• Boyuna kullanılan peri-implantlardaki kemik kaybı, daha önceden yaşanılan periodontal kemik kaybı tedavisiyle bağlantılıdır, ayni zamanda;
• İmplantlarin genel kayıp oranı ''periodontite duyarlı'' bireylerde daha fazla iken (%8), nonperiodontal bireylerde daha azdır (%3)
Deneklere ait klinik veri yeterli olmadığından hastaların derecelendirilmesi sadece radyografik yöntemlere dayanılarak yapılmıştır. Periodontal kemik düzeyi verisi kullanılarak Yaşa dayalı kemik düzeyi skoru (ArB score) hesaplanmış, bu skor deneklerin tanımı ve katmanlaşmasının periodontal hastalıklara hassasiyetine dayanılarak hesaplanmasına yardim etmiştir. Deneklerin karakterize edilmesi için Hesaplanmış ArB skorunun fizibilitesi (olabilirliği) iki ayrı son çeyrek arasındaki ortalama periodontal kemik seviyesi (%92 ye karşılık %63) ve kök uzunluğunun yarısından daha az desteğe sahip ortalama yüzde (%1 e karşılık %26) Açısından ele alinmiş ve ayni zamanda işaretlenmiş farklar tarafından ispat edilmiştir. Hugoson et. Al. Tarafından yapılmış ve deneklerin ilerlemiş periodontal kemik kaybına göre gruplandırıldığı bir çalışmada, kök uzunluğunda 1/3 ten fazla oranda toplam kemik kaybı gösteren denekler '' şiddetli periodontal hastalıklı'' olarak karakterize edilmişlerdir. Benzer şekilde, ArB skoru baz alınarak en kötü çeyreğe giren deneklerin (perio grubu) periodontit 'e duyarlı olduğu görüşü hakli gözükmektedir. Şu anki çalışmanın sonuçlarını çözümlerken akılda tutulmalıdır ki perio denekleri ortalama olarak tüm örnekler gibi kalan diş sayısı göstermesine rağmen, daha fazla söz edilen doğal diş çıkarma sırasında (dentition) yaşanan kemik kaybı, implant destekli kısmi protezlerin yerleştirilmesi koşullarında farklıklar gerektirmiş olabilir. Elimizdeki hasta örneklerindeki genel implant başarısızlık (implant kayıpları) oranı %5.2 dır. Bu oran kısmi diş sahibi olan (dentate) bireylerin yayınlanmış şu tedavi sonucu bilgileriyle karişilaştirilabilir (Naert et al. 1992; Jemt&Lekholm 1993;nevins &langer 1993;lekholm et al. 1994,1999;bahat 2000;behneke et al. 2000). Lindh et al. (1998) toplam 923 -implant destekleyen sabit kısmi protez- (implants supporting fixed partial dentures) içeren 7 çalışmanın bir meta çözümlemesini sunmuş ve yüzde 6.4 lük (5 yıllık bir zaman aralığında) başarısızlık oranı bildirmiştir.
İmplant tedavisinin periodontik olarak tehlikede olan hastalardaki neticesi birkaç vaka da dile getirilmiştir. (nevins&langer 1995; Ellegaard et al 1997). Ellegaard et al. (1997) ortalama 3 yıllık bir süreçte tek ya da kısmi şekilde implant destekli protez rekonstrüksiyon (tekrar inşaa) geçirmiş hastalarda yüzde 5 lik bir implant başarısızlığı rapor etmişken, nevins ve langer tarafından yapılmış bir çalışma 1-8 yıl takip edilmiş hasta materyalinde % 3 lük bir başarısızlık oranı rapor etmiştir. Bizim çalışmamızdaysa daha önce yıkıcı bir periodontal rahatsızlık geçirme bulgusu hasta seçiminde bir kriter rolü oynamamıştır. Hedef popülasyon önceden belirlenmiş zamanlar arasında implant destekli sabit kısmi protez ile göteborgtaki branemark kliniğinde maxilla kısmının posterior segmentlerinden tedavi olmuş kişilerdir, hastaların karakterizasyonu ise periodontal hastalık hassasiyetlerine göre yapılmış ve kişiler buna göre dahil edilmiştir. Bu yüzden, yukarda bahsedilmiş çalışmaların aksine bizim hasta örneklerimiz yıkıcı periodontal hastalıklar yönünden yüksek değişkenlik göstermektedir. İlginç şekilde, deneklerden en yüksek ArB skoruna sahip dörtte birlik kısım (perio grubu) %8 lik bir implant başarısızlığı göstermiştir. Örneğin kalan kısmi ise %4 ve %3 lük başarısızlık göstermiştir. Perio alt grubundaki yüksek başarısızlık oranı Broacard et al.(2000) Tarafından yeni bildirilmiş sonuçları doğrulamaktadır. Bizim çalışmamızda ise bu farklılık özellikle geç başarısızlıklarda belirgindir, örneğin yerleştirmenin ilk bir yılından sonra başarısız olan implantlar gibi. Yüklemeden bir yıl sonra, tüm hasta örneklerinde kaybedilen 7 implanttan 5i daha önce periodontal sorunlar yaşamış hasta grubuna aittir. Hasta örneklerindeki Köprünün takılmasından sonraki 5 yıllık sürede gerçekleşen ortalama peri-implant kemik kaybı oranı 1.8 mm olmasına karşın köprü takılması ve yükleme arasında geçen zamanda bu kayıp 0.7 mm dır. Bu yüzden, yüklemeden sonraki 5 yıl içinde gerçekleşmiş kemik kaybı ortalama 1.00 mm bulunmuştur ve bu oran maxilladaki implant destekli protezler üzerinde yapılmış diğer çalışmalar (0,8 mm jemt ve lekholm; 1993 tarafından doğrulanmıştır. Bununla birlikte; hastalardaki ortalama peri-implant kemik kaybı sinirli bir önem arz ederken verilerimiz göstermektedir ki peri-implant kemik kaybı ve implant tedavisinden önceki doğal diş yapısındaki ilerlemiş periodontal kemik kaybı arasında istatistiksel olarak kayda değer bir bağ vardır. Buna bağlı olarak; en yüksek değerdeki ArB skoruna sahip bireyler, yükleme ve 5 yıllık periyot arasında geçen zamanda diğer bireylerden ortalama %30 daha fazla kemik kaybı göstermiştir. Kalan dişlerin yapısındaki gerileme ise yetersiz radyografi verisi nedeniyle değerlendirilememiştir. Buna ek olarak, plak nedeniyle oluşmuş lezyonların periodontal/periimplant destek tedavisiyle kontrol edilip edilemeyeceği yetersiz klinik veri nedeniyle bilinememektedir. Weyant ve Bert(1993) ve lindquist et. Al. İn da çalışmalarında gösterildiği gibi, sigara içmenin peri-implant kemik kaybındaki rolü sigara içiş rutinleri asla kaydedilmediği için sağlıklı olarak belirlenememiştir. Bu nedenler dolayısıyla, periodontal doku bozunması ve periodontal kemik kaybı arasındaki gözlemlenmiş bağ daha ileride gerçekleşebilecek derinlemesine çalışmalarda kullanılmak açısından umut vaat etmektedir.

Abstract
This study presents oral rehabilitation with osseointegrated implants in partially edentulous patients treated for generalized severe adult periodontitis. Five female patients aged between 31 and 44 received a total of 36 implants and were observed for 1 year after insertion of the superstructure. Three months before implantation, venous blood samples were taken from the patients and five periodontally healthy controls, and the serum examined with highly sensitive ELISA test kits for interleukin 1 beta (IL-1 beta) and interleukin 6 (IL-6). Clinical examination covered the plaque index (PI) and gingival index (GI) at teeth and implants plus probing depth (PD) and clinical attachment level (CAL) at the teeth. Microbiological evaluation of teeth and implants was performed by dark-field analysis, and DNA analysis was performed in addition 1 year after insertion of the superstructure. Radiological controls of the teeth were carried out with standard single-tooth films in the 2 years preceding implantation and at baseline. Following implantation, further controls of the teeth and implants were undertaken immediately after insertion of the superstructure and 1 year thereafter. The immunological examination revealed that the IL-1 beta (0.22 /- 0.2 pg/ml) (means /- SD) and IL-6 (2.27 /- 2.8 pg/ml) level was slightly, but not significantly, higher in the patients than in the control group (IL-1 beta: 0.06 /- 0.06 pg/ml and IL-6: 0.64 /- 0.2 pg/ml) (P > 0.05). The clinical results show that the GI at the teeth rose slightly from 0.0 to 0.2, and at the implants from 0.3 to 0.4. The PI rose slightly from 0.3 to 0.7 at the teeth and from 0.2 to 0.9 at the implants. Neither the GI nor the PI revealed any significant difference between teeth and implants. Clinical attachment loss at the teeth was minimal at 4.7 to 4.8 mm. Comparison between the teeth and the implants revealed no essential difference in bacterial flora; neither Actinobacillus actinomycetemcomitans nor Porphyromonas gingivalis was recorded at any location. Small quantities of Prevotella intermedia were detected at the teeth and implants of one patient. Radiological evaluation 1 year after insertion of the superstructure revealed a mean bone loss of 0.62 mm at the implants. The bone loss at the teeth during the same period was 0.3%, whereas it had been 1.5% in each of the 2 previous years. These results suggest that there is only a slight difference between the periodontal and peri-implant areas in patients with generalized severe adult periodontitis. The full potential for implants in these patients, however, needs to be shown in controlled longitudinal studies.

Bu çalışma, yaygın ağır periodontit nedeniyle tedavi görmüş osseoentegreli implantlara sahip kısmen dişsiz hastaları konu almaktadır. Çalışmada; Ortalama 31 ve 44 yaşları arasındaki 5 kadın hastaya 36 implant yerleştirilmiş ve bu hastalar Üst yapının yerleştirilmesinden sonra 1 yıl boyunca izlenmiştir. Yerleştirmeden 3 ay önce hastalardan ve 5 sağlıklı kontrol grubu elemanından venöz kan örnekleri alinmiş ayni zamanda serum yüksek derecede duyarlı ELİSA test kitleri tarafından interleukin 1B (İL-1B) ve interleukin 6 (İL-6) için incelenmiştir. Yapılan klinik inceleme ise plak indeksini (plaque index) (Pİ), diş eti indeksini (gingival index) (Gİ), implantlar ve sondalama indeksini (PD) ayni zamanda Klinik Bağlanma indeksini (CAL) kapsamaktadır. Dişler ve İmplantlardaki Mikrobiyolojik inceleme ise dark field analizleriyle yapılmış olup ek olarak üst yapının yerleştirilmesinden bir yıl sonra DNA analizleri gerçekleştirilmiştir. Radyolojik kontroller; İmplantlarin yerleştirilmesinden 2 yıl sonra tek-diş filmleri kullanılarak yapılmıştır. Yerleştirmeyi izleyen bir yıl içinde dişlerin ve implantlarin daha ileri kontrolleri yapılmıştır. İmmünolojik incelemede İL-1B (0.22 - 0,2 pg/ml) (ortalama - SD) ve İL-6 (2,27 _ 2,8 pg/ml) düzeyinin düşük ama çok belirgin olmayan oranlarda hasta grubunda kontrol grubuna göre daha yüksek olduğu bulunmuştur. Klinik incelemeler göstermektedir ki dişteki Gİ 0,0 dan 0,2 ye, implantlardaki Gİ ise 0,3 ten 0,4 e tırmanmıştır. Pİ da ise durum şöyledir: dişlerde 0,3 ten 0,7 ye, implantlarda 0,2 den 0,9 a. Gİ ya da Pİ diş ve implantlar arasında belirgin bir fark göstermemiştir. Dişlerdeki Klinik bağlanma kaybı 4,7 den 4,8 e çıkmasıyla minimumdadır. İmplantlar ve dişlerin klinik olarak karşılaştırılması sırasında bakteri florasında önemli farklar görülmemiştir; hiçbir lokasyonda Actinobacillus actinomycetemcomitans ya da Porphyromonas gingivalis kaydedilmemiştir. Sadece tek bir hastanın implant ve dişlerinde Prevotella intermedia ya rastlanmıştır.Üst yapının yerleştirilmesinden bir yıl sonra yapılan radyolojik değerlendirme, implantlarda ortalama 0,62 mm lik bir kemik kaybı açığa çıkarmıştır.Ayni zaman aralığında dişlerdeki kemik kaybı %0,3 iken önceki 2 yılda %1,5 tir. Bu sonuçlar yaygın ağır periodontite sahip hastalarda periodontal ve periimplant bölgeleri arasında sadece çok küçük farklar olduğunu önermektedir. Fakat İmplantlarin tam potansiyelinin gösterilebilmesi ancak derinlemesine yapılacak çalışmalarla mümkün gözükmektedir.
